Need to replace Cameron with Lord Howard (or someone of similar stature) immediately - new cabinet of Leaver unity to steady the constitutional position of the country and giving the Tory party time to regroup and have a proper leadership race.

My guess however is that it will go to Parliament - the only way that Boris can at least try to share the blame (didn't work that well for Blair/Iraq tho). A tough call for lots of remain MPs if the economy is tanking by the autumn!

How Mr Corbyn could end Cameron’s Premiership
Were David Cameron to campaign for Remain and Leave won then that would almost certainly trigger his immediate resignation/removal as Conservative Party leader and thus First Lord of the Treasury, so the fate of the Prime Minister could well be in Mr Corbyn’s hands. Over to you Mr Corbyn.
Yesterday’s ICM poll gave Remain a 4% lead when you add in the potential for a very disunited Labour party on the EU referendum, Remain supporters should be very nervous about the outcome.

Given all the claims of doom by Remain prior to the vote this is much more in line with reasonable expectations.
